PRIVIT Checklist - Fall 2023
- Join the Team titled, "2023 Fall New/Transfer/Re-Admitted" (password: NEW)
- ATHLETES - Join the appropriate sports team.
- Complete the general information form.
- Complete the emergency medical Form.
- Complete the student medical form.
- Upload your immunization records or approved accommodation form.
- Upload your insurance card (front and back).
- ATHLETES - Complete Sport Required Forms
- Email accommodation forms to wchealthcenter@wilmington.edu for approval. Once the accommodation form is approved, the form will be emailed back to you. Upload your approved accommodation form.
